What happens after you submit

Submitting a request is not the same as a confirmed booking. A member of the clinic team reviews every request, checks which specialist and which clinic day suits your concern, and then contacts you to agree a time. If your symptoms change, or become severe, while you are waiting to hear from us, seek urgent local medical care rather than waiting for a reply.

You will see a reference number as soon as the form is submitted. Keep it — you can quote it in any message, and if you create a patient portal account you can use it to link this request to your account and follow its progress.

What the form asks, and why

  • Your concern — so the request reaches the right specialist and enough time is set aside
  • Contact details and preferred method — so the team can reach you the way you prefer
  • Preferred dates — a starting point for finding a time, not a guarantee
  • Whether you have been seen before — so your record can be found
  • Documents — previous results and letters, so your specialist can review them beforehand

Privacy and consent

Everything you share with Quantum Urology Clinic is treated as confidential health information. Only the members of the clinic team who need your details in order to arrange or provide your care are able to see them, and notifications we send you never contain clinical detail — only a reference number and a neutral subject line.

You are asked to consent to your information being used to arrange your care. Marketing consent is separate, optional, and can be withdrawn at any time without affecting your care.

If your symptoms change while you wait

Contact the clinic if your symptoms worsen. Seek urgent local medical care — do not wait for a reply here — if you cannot pass urine, have severe pain, a high fever with urinary symptoms, heavy bleeding, or feel very unwell.

Frequently asked questions

Is this a confirmed booking?
No. It is a request. The clinic contacts you to agree a time, and only then is your appointment confirmed.
Do I need an account?
No. Requests can be submitted without one. A patient portal account simply lets you track status, messages and documents in one place.
Can I upload my test results?
Yes. Documents are stored privately and are visible only to the clinic team involved in your care.
What if I need to cancel?
Contact the clinic as early as you can so the appointment can be offered to someone else.
